Why is caregiving so hard in America? The answers emerge in a new film A new documentary on PBS shows what it's like to care for adult family members and recounts the history of caregiving policy in the U.S., revealing why those caring for family are often on their own.Caregiving traces the history — and unique challenges of — caring for family members in the U.S. In the documentary, viewers meet caregivers like Malcoma Brown-Ekeogu, who now helps her husband, Kenneth, with even his most basic needs, like walking and bathing. Many people caring for a sick or elderly relative are shocked to find out that Medicare does not cover the cost of a nursing home or subsidize care at home — the cleaning, driving, and helping with meals and dressing that so many families take on. Private health insurance doesn't pay either. In the United States, caregiving is largely a private matter rather than a public concern. Americans caring for elderly or disabled adults cobble together help from nonprofits, community groups, church, friends and family — and even though there are 53 million of them, often feel intensely alone.

Home Care: Family Plus Formal Help Makes a Difference - Penn LDI

In LDI’s Norma B. Coe’s new study, she found that a combination of family and formal care at home led to better health outcomes than either approach alone. Norma Coe is a rare breed, a renowned economist who studies long-term care and served as a front-line caregiver for both of her husband’s parents. Coe’s father-in-law lived with her family for six years, and she fortified her own caregiving with the formal care of experts. In her latest study, Coe returned to caregiving, and tested whether the combination of family and formal care at home—a growing but still unusual arrangement—led to better health for individuals than either approach alone. While the very poor can get long-term care through Medicaid and the wealthiest can pay out-of-pocket, the middle class enjoy few options and must find the money or impoverish themselves before Medicaid coverage starts. Family members often fill those gaps, saving the government billions of dollars at the expense of the caregiver’s earning power. One showed how caregiving leads women to earn less than non-caregivers. The other found that caregivers’ depressive symptoms rose over time. Coe and her team launched the most recent study after noting the growth of people in the community with disabilities who had both family and formal caregivers.
